Delcastle families, the athletic forms process just got simpler. Beginning with the 2026–27 school year, all DIAA sports physicals and required athletic paperwork move online to Rank One, the secure system DIAA uses to collect and verify student-athlete forms.
No more dropping off paper packets at the front office. No more wondering whether a form made it to the right person. Once you set up your account, everything lives in one place, and you'll always be able to see exactly what's complete and what still needs your attention.
Here's everything you need to know to get your athlete cleared to play.
The most important change: don't submit physicals to the school
In the past, families completed a paper DIAA physical and dropped it at the High School Office. That process has ended. Physicals turned in to the school will no longer be accepted.
Instead, you'll upload the required page from your physical and complete the rest of the forms directly in Rank One. The remaining pages of the physical packet stay with you and your medical provider.
Step 1: Get the physical
Your athlete still needs a current sports physical completed on the official DIAA Sports Physical form.
- Download and print the DIAA form and bring it to your appointment.
- The physical must be dated on or after April 1, 2026 to be valid for 2026–27.
- The physician or nurse practitioner who performs the physical must sign the Medical Card (page 5) — that's the page you'll upload to Rank One.

Step 2: Create your Rank One account (just once)
Head to the NCCVT Rank One portal and create an account. You only need to do this one time.
- Add your student by entering their first name, last name, and date of birth to claim them.
- Have more than one athlete? You can add multiple students to a single account.
- Incoming freshmen will become available in the system starting July 1, 2026.
Step 3: Complete all six forms online
This is the heart of the process. All six forms must be finished and signed electronically for your child's physical to count. Do not skip any.
- Transfer — Everyone must answer the transfer question, even if your child has never transferred. Do not skip it.
- Participation Interest — Select the sports your athlete might participate in this year.
- Medical Card — Upload page 5 of the DIAA physical, signed by your provider.
- DIAA Acknowledgment Form — Complete and sign online.
- Consent Form — Complete and sign online.
- Emergency Card — Add emergency contacts for your athlete.
Step 4: Confirm everything shows "Complete"
Before you're finished, double-check that each of the six forms displays a Complete status. A physical is only valid once all six are done. A missing or skipped form means your athlete isn't cleared to try out or play.

A few quick reminders
- Don't bring physicals to the school office. Submit everything through Rank One.
- Start early. If your athlete plans to try out in the fall, get the physical and forms done over the summer so there are no last-minute surprises.
- All six forms, every time. A physical isn't valid until everything shows Complete.
