DIAA Sports Physicals for 2026-2027: Now Submitted Online Through Rank One
 

To try out or participate in any sport, the Delaware Interscholastic Athletic Association (DIAA) requires a current sports physical. Beginning with the 2026–27 school year, all physicals and required athletic forms are completed and submitted online through Rank One.

⚠️ Do not submit your DIAA physical to the school office. Paper physicals are no longer accepted. Everything is completed and uploaded at Rank One.

 

What You Need to Do

1. Get the physical. Download the official DIAA Sports Physical form and have it completed by a medical provider on or after April 1, 2026 for the 2026–27 school year. The physician or nurse practitioner who performs the physical must sign the Medical Card (page 5) — that's the page you'll upload.

 

2. Create your Rank One account. Set it up once at the NCCVT Rank One portal. Add each of your students using their first name, last name, and date of birth. You can add multiple students to one account. (Incoming freshmen become available July 1, 2026.)

 

3. Complete all 6 forms online — don't skip any.

  1. Transfer — Everyone must answer the transfer question.
  2. Participation Interest — Select the sports your athlete might play.
  3. Medical Card — Upload page 5 of the DIAA physical, signed by your provider.
  4. DIAA Acknowledgment Form — Complete and sign online.
  5. Consent Form — Complete and sign online.
  6. Emergency Card — Add your athlete's emergency contacts.

4. Confirm "Complete." Make sure all six forms show a Complete status before participation. A physical is only valid once every form is finished. A skipped or missing form means your athlete isn't cleared.

 

Helpful Tips

  • Download the Rank One Sport app to manage forms from your phone and receive team updates and communication once your student is rostered.
  • Start early, especially if your athlete plans to try out in the fall.
